Models and theories of case management, intake assessment, service planning and coordination, monitoring, documentation and use of technology, and termination of services. Continuum of interventions (i.e., prevention, treatment, maintenance, aftercare) is addressed within the context of a spectrum of service settings and ethical and culturally appropriate responses.

Learning Outcomes
* Articulate how human services values, attitudes, and ethics are integrated in case management.
* Compare and contrast mental health, social, medical, medical/social, wrap-a-round, individually tailored services, and strengths-based models of interventions.
* Analyze the relationships between basic case management tasks including: intake and termination, needs assessment, action plans, file documentation, resource location, professional consultation, referrals, feedback loops, administrative codes, law, ethical concerns, client and systems advocacy and use of technology.
* Design a process to identify, assess, and periodically evaluate resources for client referrals.
* Compare and contrast mental health, social, medical, medical/social, wrap-a-round, individually tailored services, and strengths-based models of interventions.

Many weeks we will be involved in skill building or collaborative exercises (lab) that cannot be made-up with the same quality of learning that takes place in the classroom. T
Online Dialogue
Students will be split into four discussion groups. You should read everything posted in your assigned group. You may read and post in other groups, but your grade will be based on the postings in your assigned group. You should post two substantive comments each week on two days.
Case Study
Students will analyze a case study with a group of students and design an intervention from the intake interview through the final assessment. Students will individually analyze a separate case study to design an intervention from the intake through the final assessment.
Skill Building Exercises
Each student will practice completion of paper work and managing case files over a 4-week period.
Complete a case file using another student as clients. Including an intake interviews, assessment, referrals, case notes, action plans, and discharge plan. During case managing each participant will be observed and given feed back on how to improve their case management skills. At the end of the 4-week period, each student will write a paper summarizing and assessing their learning experience.
Intervention Reports
Each student will research and write a report on a specific intervention for a specific population. The report should include (a) demographics regarding the population served, (b) the types of services provided, (c) how clients qualify for services, (d) laws or policies that affect services, (d) the range of service providers, and (e) how services are funded. Are there services needed that are generally not provided?
Presentation & Panel Discussion
Each student will give an overview of their report. Discussion will be continued online during the last two weeks of the quarter.
Final Exam
There are four learning outcomes for this course. The final exam will be tied to the four learning outcomes.
